 MEDICAL INSURANCE: (Available after 30 days)

Option A

Carrier:  Blue Cross/Blue Shield (BCBS)

$425 deductible, 80% coverage for the 1st $10,000, 100% thereafter.  Individual out-of-pocket max $2,200.  Family out-of-pocket max $4,400.

Employee only = $42.95 per pay

  • Employee plus children = $103.23 per pay  
  • Family = $126.75 per pay

All services (with the exception of contract services) provided at an HMA facility paid at 100%.

Option B

Carrier:  Blue Cross/Blue Shield (BCBS)

$775 deductible, 70% coverage for the 1st $8,000, 100% thereafter. Individual out-of-pocket max $2,600. Family $5,200. 

  • Employee only = $36.65 per pay
  • Employee plus children = $79.08 per pay
  • Family = $101.55 per pay

All services (with the exception of contract services) provided at an HMA facility paid at 100%.

Preventive or Routine Physical (includes Well Baby/Well Child) NO copays.  100% covered not subject to deductible.
Emergency Room Copay: $125.00 copay waived if admitted.  Facility charges including room & board $250 copay.

PRESCRIPTION PLAN:  (Available after 30 days)

Pharmacy Card Program through CareMark is included with your medical election of option A or option B.

DENTAL INSURANCE:  (Available after 30 days)

Carrier:  Dearborn National

$50 Deductible per individual up to $150 per family, 100% coverage for basic and preventative services, 50% for major services.
Annual benefit maximum of $1,500 per person.

          • Employee only = Dental only $12.85 per pay.  Dental + Ortho $14.03 per pay
          • Employee + children = Dental only $22.13 per pay.  Dental + Ortho $27.64 per pay
          • Employee + Spouse = Dental only $24.48 per pay.  Dental + Ortho $28.92 per pay
          • Family = Dental only $31.32 per pay.  Dental + Ortho $37.70 per pay

VISION SERVICE PLAN:  (Available after 30 days)

Carrier:  Vision Service Plan (VSP)

VSP eye exam every 12 months covered in full from a VSP doctor after $10 co-pay.  Single vision, bifocal and trifocal lenses $25 co-pay.  $130 towards purchase of frames every 24 months.  Contacts at a $105 allowance.

  • Employee only = $3.63 per pay
  • Employee + Spouse = $5.91 per pay
  • Employee + Children = $6.06 per pay
  • Family = $9.39 per pay

LIFE INSURANCE BENEFIT:  (Available after 30 days)

Employee Life Coverage through Prudential, with two options:

Basic Life:  1X annual earnings to a maximum of $500,000 at no cost to the employee.
Optional Life:  2X annual earnings, cost determined by employee annual earning.
Dependent Life:  (Available after 30 days) Dependent life coverage through Prudential for legal spouse and dependent children only.

  • Life-Spouse Coverage = $25,000
  • Life-Dependent Child(ren) = $10,000
  • Cost = $4.52 per pay

SHORT TERM DISABILITY:  (Available after 30 days)

STD coverage through Prudential.
Cost = $1.44 per $100 of monthly covered payroll.

LONG TERM DISABILITY:  (Available after 30 days)

LTD coverage through Prudential available to employee only and calculated by rate of pay and age.

RETIREMENT PLAN (401-K)

(All newly hired/newly eligible employees will automatically be enrolled in the Plan for 4% pre tax contribution unless you make a different election.  You have 45 days from the date Prudential is notified of your employment to decline your participation in the Plan.)
Retirement plan through Prudential Financial with automatic enrollment of 4% once required days or hours worked are met.

Employee Contributions:

FSA-Flex Corp:  (Available after 30 days)

FLEXIBLE SPENDING ACCOUNT:
Full-Time associates can contribute pre-tax dollars on an annual basis to use for healthcare and dependent care expenses.  Per Healthcare Reform, over-the-counter drugs will be eligible for reimbursement ONLY when accompanied by a doctor's prescription (efft. 1/1/11).

MEDICAL ACCOUNT:
Annual amount is limited to $5,000

DEPENDENT CARE ACCOUNT:
Annual amount is limited to $5,000 or if married, but filing separate tax returns, limit is $2,500.

Allstate (Available after 30 days)

CANCER INSURANCE: Upon diagnosis with cancer, a policy holder can receive financial benefits that supplement

CRITICAL ILLNESS INSURANCE: Upon diagnosis with a covered critical illness, a policyholder receives an immediate financial benefit.  Associates may choose coverage amounts ranging from $10,000 to $30,000.  Coverage is also available for your dependents.

Unum (Available after 30 days)

WHOLE LIFE INSURANCE: You can select the amount of coverage that you would like to have for yourself, your spouse, your children and your grandchildren.  Coverage is portable.  You can take it with you if ou change jobs or retire.

ACCIDENT INSURANCE: The financial payment correlates with each accident, and the money can be used any way you choose.  It helps pay the extra expenses that add up as a result of each injury, even your ER copay.  Coverage is also available for your dependents.
